Please fill out the Natix Oral Biopsy requisition form and email it to reception@natix.ca .

Your specimen and a hard copy of the requisition should be placed in a sealed ziplock-type bag and placed in the Biospy Dropbox at Suite 125. Any specific questions can be forwarded to Dr. Firoozeh Samim at fsamim@natix.ca .
